Back Pain Relief: Why Your MRI Isn't a Crystal Ball

Living an active lifestyle in the Bay Area is a core part of who we are. Whether you are running the Dipsea trail, cycling through Marin County, or pushing your limits in the gym, you rely on your body. So, when back pain strikes and a doctor hands you an MRI showing a "disc bulge," it is easy to panic and assume your high-performance days are over.

But as a physical therapist, I'm here to tell you that your MRI is not a crystal ball.

You Are Not Your MRI

A massive 2015 study in the American Journal of Neuroradiology examined individuals who experienced zero back pain. They discovered that 50% of healthy 30-year-olds had disc degeneration, and by age 50, up to 80% of pain-free people had bulging discs.

For athletes, this is crucial news. It means an ugly MRI doesn't mean you have to stop performing. Diagnosing pain purely based on an MRI image is heavily flawed. Instead, we need to assess how you actually move.

The Athlete's Movement Inventory

At Dragon Physical Therapy, we turn our clients into "movement detectives". We stop looking at the MRI and start tracking patterns of behavior. We break your daily movements into three columns:

  • The Green Zone: Activities that reduce your pain, like lying flat or walking.

  • The Neutral Zone: Movements that make no difference.

  • The Red Zone: Your pain triggers, such as bending over or carrying heavy loads.

By finding the common themes—like whether your body prefers extension (standing) or flexion (sitting)—we can create a customized manual for your recovery. If your pain originates from the lower back, pelvis, or hips, this framework will point us directly to the root cause. We focus on emulating your Green Zone activities and temporarily neutralizing the Red Zone so you can heal while staying active.
